Should I integrate directly with Visa and Mastercard? Is this expensive? Or should I just use PayPal? Will people get annoyed and leave if they have to go through PayPal to purchase an item?

Don't forget to add your website into PayPal Shops (go to PayPal, click the Shops link at the bottom). It's a great way for people looking to pay for something with PayPal to find you. The downside of PayPal is that the cut PayPal takes can feel fairly substantial, especially on credit cards.
Google Checkout is new, and they aren't taking any percentage on sales until 2008, and even then their rate is going to be less than PayPal's. Find them at http://checkout.google.com/
I have processed about 6 transactions so far with no problem. My clients have had good things to say about it too. They tell me it was quick and easy.
I'm sure you're also taking into account what type of business you have. If you sell an extremely high-end product, you might want to go with a different kind of credit card processor, and a type that will allow you to accept payments over the phone. I've used http://www.propay.com in the past, and it was convenient and easy to work with (but a little too expensive for my needs)...it might be right for you though.
