-Your loan was made before the end of the fifth year of qualifying teaching.
-The school must be public or private nonprofit.

Perkins Loans can be up to 100% of the loan and does not require 5 years of teaching nor do you have to teach full time. You apply for the forgiveness at the school to which you took the Perkins Loan as they administer the loan.
You can also check with the state that you move to and see if they offer any loan forgiveness programs.
